I believe at the time of his death he was planning on campaigning into Arabia. I think this would have been very interesting. The area the Romans referred to as Arabia Deserta would have been very difficult to successfully invade and control as its inhabitants were mostly nomadic. The same would go for a large part of Arabia Petraea. So Alexander's success in Arabia would be by no means a definite thing, and anything after that would be complete speculation.
And I see that while I was writing about "complete speculation" the thread was moved to Speculative History, how fitting!

Perhaps Alexander would have went west, against the Illyrians and other western Balkans peoples. If he was greeted with success there, an invasion of Italy? It's also possible that he would have launched another campaign against India.

If you believe the letters he supposedly wrote then he'd have gone into Arabia and after that into the Western Mediterranean, so that'd be Carthage, Italy and Iberia. I don't think anything would put up much resistance to be honest, I suppose Carthage may hold out for a while and Iberia may take a while to suppress with the guerrilla warfare but it wasn't anything he hadn't seen and beaten before.

A lot of ambitious characters in the Macedonian Empire; given how they acted after Alexander's death, I would be shocked if one or two of them weren't trying to stick a knife in their King's back.

He would most likely be killed or betrayed and killed by one of his generals or even his own son, Alexander II.

If he was smart he'd settle down for a few years, get a wife, get an heir, and make sure his empire won't collapse, however, knowing him, this is no what he would have done. He either would have gone after india again, invaded arabia, or move west, and smash rome before it was an empire, destroy carthage, take over iberia, and possibly invade germania and gaul.
